At my internship, sysadmin had everyones (colleagues) password stored in a excel and did not lock his computer/office when he went to get coffee. At least his excel had a password on it but left it unlocked.
At my previous job, I got the CTOs credit card info & PIN code in my first week there.
Current job: password of a soon-to-be-legacy system of our customers is stored in plain text -

Overheard one of our FS accountants telling another one her dog with cancer was going to be OK because she (the dog) was a <insert zodiac sign I don't remember>.
The other said "Oh yes...she's going to be fine." -
Interpretations. Issue mostly at my first job, but was visible in other companies for which I have worked. When the documentation unclear I am usually surrounded by people who like to debate / interpret things.
The solution, however, is very simple - you just ask the person who made the documentation for clarification.
Yes, YMMV, but this example relates to documentation regarding rules for publishing on some platform, so if anything is unclear - yes, definitely ask them for clarification. -

people just plain don't get back to you about _anything_ and mass miscommunication... questions are basically not allowed, deadlines are inconsistently communicated, etc
wait new job? I stayed there for quite a while. sigh
if only anyone knew anything or talked. everybody was just plain scared or stupid. dunno
... and then turns out the culture shifted into a blame culture since I last was there. so you got blamed for things nobody ever communicated to you, and they had no interest in learning that maybe they should communicate things to make all this work without friction then ??? @_@
