Top center and bottom right may also have some bread in the top right of the images.

At this point, they could simply allow 1% of users register at random, bots or not.

I think have a deeply buried mental scar. Not even sure if this can actually have been the reality, but... weren't some early captchas all about case-sensitive letter and number recognition, with characters of varying sizes? Meaning you still had to decide if you were looking at a large lower-case 'x' or a small upper-case one? No access until you got it right.
