Anyone know a form builder that works on top of angular (or pure js/jquery) so that a user can use it to modify certain parts of a web app?

Haha its not as wrong as it sounds (i hope).
Its for a school project where our client is a clinic (im not sure if its the right word, its a place where you go to have them extract blood and take urine samples for doing studies, for example a diabetes test) and one of our requirements its that they should be able to add information about a study (name, samples to be taken, different field for each "printed study") and then define a "form" for printing and filling in the results of the study for a certain patient. So we need something that is simple enough for them.
I hope this clarifies what ive asked.
