I can't even begin to go this rant road. I might get banned. So feel you though.

A friend of mine who worked at the biggest streaming provider in my country asked me, off the record, if I could code a simple UI form so he didn't need to compose a custom sql script everytime for a routine task.

I made a ~150 line php file with full access to the customer db which ended up being used by all their sysadmins, and they never even checked the code before running it.

I would never trust myself with that.
