    That rule number 12 is questionable. In PostgreSQL when someone inserts rows with manually set primary key (that has auto increment turned on) then sequence for that primary key will not be updated. So next time someone inserts rows while using said sequence it will result in "duplicate row" error. But sequence will increment by 1 regardless if query succeeds or fails. Attempt to execute said query enough times and voila - sequence catches up and inserts finally work.
