I've often wondered this as well.

Generally I let it go to voicemail and call them back, explaining I was indisposed or otherwise unable to answer. I've only had a few people get angry, and they were assholes anyway.

I took a call from my current job while taking a shit but it was bc I didn't recognize the number and assumed it was a spam call (I had been getting the calls from unknown numbers with the same area code I'm in).

I had it on speaker whenever I picked up, but that lasted 0.5 seconds whenever I realized it wasn't a spam call.

take the call and ask them to call (or tell them that you'll call back, whichever makes more sense) literally in 5 minutes. done that several times. in my opinion, it shows that you're interested, were ready on time, but something unexpected came up, but you're still able to communicate promptly and let them know.

at least that's how i interpret it from my side, and would interpret it from the other, if i was the interviewer.
