Seems like this could've been easily Googled...
But anyway, you need to buy the domain from a registrar, and then forward it to wherever your server lives. Your host need not be the same as your registrar. -
If you need a registrar that's efficient and easy use bluehost. I've heard host gator is pretty simple to use and ramp up as well.

If you need dedicated servers you can check hetzner's -

You need to setup apache. Put your web files inte the /var/www folder...
Then, open your ports on the router, your website is reachable by your ip.
Now, to connect that IP to an url, you need a DNS-Provider
